Please verify or update the following: • Full Name • Mailing Address • Phone Number • Email Address • Date of Birth • Branch of Service • Conflict/Era of Service We also need a copy of your DD-214 or discharge orders on file. Even if you have provided a copy in the past, we need you to provide another copy at this time so we can make sure our current records are complete and up to date. This is NOT a spam request.
